Add a way to add notes for tos violations

I think this would be a really good idea, especially with tos violations of real people. I'm sure most mods do not know of every celebrity in the world. Adding this will allow people to say who the person is if any embedding is deleted or just make it easier if the name of the person was put into the spaghetti of a generation.
